Appointments are generally available every week day, depending on the location and the practioner you will be seeing.

When you come for your appointment please remember to obtain and bring the following:

Current Insurance card (s)
A Referral if required by your insurance carrier
A list of your current medications, vitamins and supplements
If you are coming to us from out side the Partners Network, we well note have access to your medical records. Please bring with you, records that are relevant to your visit. When bringing Imaging, Xray, Ultrasound MRI, etc – DO NOT bring films or CD’s, REPORTS ONLY
